Primary Care Extended Access Survey 2017

High Weald Lewes Havens Clinical Commissioning Group (HWLH CCG) are asking patients to complete the following

HWLH CCG plans and buys healthcare services for the local population. National funding will be made available from 2018 onwards to improve access to GP services. This includes making sure that everyone has easier and more convenient access to GP services including appointments on weekday evenings and at weekends if required. We’d like to find out patient views on improving access to GP appointments/primary care services in evenings and at weekends. This survey will give you the opportunity to tell us about when you would like to be able to access GP appointments, about the kind of services you might want to use seven days a week, about where you’d be willing to travel to access them and alternatives to face to face consultations. The survey is being carried out by Quality Health on behalf of NHS High Weald Lewes Havens CCG. Quality Health will process your answers in confidence. All name and address information will be kept separately and will be destroyed within three months of completion of the survey. Your personal data are held in accordance with the Data Protection Act 1998 and the NHS Confidentiality Code of Practice. If you would like more information about the survey or need help to answer the questions, please call the FREEPHONE helpline on 0800 783 1775 and they will do their best to help. The line is open between 9am and 5pm, Monday to Friday and there is an answerphone at all other times where you can leave a message

The Surgery is in Southover Road, down St Andrew’s Lane or Station Street from the High Street. It is a flat walk from the bus station along Friars Walk and Lansdown Place. There is limited waiting parking in Southover Road, Friars Walk and St Andrew’s Lane. There are car parks in Mountfield Road, or behind the Magistrate’s Courts, or at the Railway Station.

For patients with health or mobility issues who find it difficult to use public transport CTLA Community Transport offers a Dial  a Ride service to pick you up from home and take you to and from the Surgery

There are parking bays marked "disabled" on the road opposite the surgery.
